What can Ascension Care Management do for me?

Our team of experts help you:

• Get Support you need to manage health concerns like stress, as well as conditions like diabetes, asthma, high blood pressure, kidney disease, arthritis, CHF, heart disease, depression and more. You can also get support for your overall health and well-being that helps you set health goals and meet them.
• Get Empowered with information about your overall health, a new or existing diagnosis, your care plan, recovery, benefits appointment scheduling and resources available to you such as medical transportation and medication assistance.
• Get Direction on billing issues, your medical plan, and how to handle complex healthcare coordination. We can remove stress by helping you understand and solve benefits issues.

We are a benefit that your medical plan has included to you at no additional charge to improve your overall well-being. Please reach out to us with your healthcare concerns.

Does it cost anything?

No! Our services are free to you as a benefit of your medical plan or Medicare.

When are you open?

We are available from 8 a.m. to 5 p.m. Central, Monday through Friday.

How do I contact ACM?

Do you work with my doctor?

Yes, we work hand-in-hand with care providers and facilities to provide extra care outside of your doctor's office. Your doctor may request for us to help you follow a care plan, work with you to meet health goals, provide medication reconciliation, or help you get other resources you need to best manage your health.

How do you get paid?

Your employer or Medicare pays us to help ensure the best supported and coordinated care for you. If you're a Member, our services are offered to you at no additional cost. 

How do I become a Member?

Currently our services are offered through your employer-sponsored health insurance plan or your Medicare-participating care provider. At this time you cannot sign up or buy our services as an individual.

My family already helps me out. Why do I need you too?

Your care team includes experience nurses and healthcare professionals that can offer additional services to the care you may already receive at no cost to you. We also know that caring for a loved one can be a confusing and time consuming, and we are there to offer support if your family needs it. Please let them know about these services available to you so that they can reach out to us if they have questions or need help. Someone from ACM visited me in the hospital. Is that normal?

We work hand-in-hand with care providers and facilities. If you are admitted to the hospital, a member of your care team may visit your bedside to see what we can do for you.

What is Health Coaching?

Health Coaching is personalized, one-on-one guidance offered by healthcare experts to help you make the changes you want to make in your life.

I'm turning 65. Can I keep your services or my insurance?

Turning 65 means you have many options for care and coverage, and our healthcare experts can help you make the right decision for you and your family.
