Yes, we work hand-in-hand with care providers and facilities to provide extra care outside of your doctor's office. Your doctor may request for us to help you follow a care plan, work with you to meet health goals, provide medication reconciliation, or help you get other resources you need to best manage your health.
Your employer or Medicare pays us to help ensure the best supported and coordinated care for you. If you're a Member, our services are offered to you at no additional cost.
Currently our services are offered through your employer-sponsored health insurance plan or your Medicare-participating care provider. At this time you cannot sign up or buy our services as an individual.
Your care team includes experience nurses and healthcare professionals that can offer additional services to the care you may already receive at no cost to you. We also know that caring for a loved one can be a confusing and time consuming, and we are there to offer support if your family needs it.
